Two recent passport photos

Bank statement

Travel itinerary
Confirmed flight bookings in and out of Afghanistan
Proof of accommodation
Hotel bookings or invitation letter from a host
Travel insurance
Recommended — covering medical and emergency evacuation

How to Apply
